Qualified Professional, I/DD - Group Homes

Location: Taylorsville, NC

Abound Health is looking for a full-time Qualified Professional to support our clients residing in our Group Homes. The Qualified Professional is responsible for upholding Abound Health's mission, vision and values by providing support, services, education and compassion to Service Recipients receiving Innovations Waiver or State Funded Services. As the Qualified Professional supporting our Group Homes, you will be responsible for visiting our group homes for supervision when clients are present, which may extend their work hours beyond 5pm on occasion. This flexibility is essential for assisting with day coverage, accompanying clients to doctor visits, addressing client behaviors that arise after regular hours or on weekends, and participating in staff training on clinical issues. Abound Health operates group homes in the following locations:

  • Taylorsville, NC
  • Hudson, NC
  • Lenoir, NC
  • Marion, NC

Key Responsibilities:

  • Review assigned participant’s service/grid notes within a timely manner
  • Review all verification/documentation of services for appropriate signatures and documentation per the NC DHHS Records Management and Documentation Manual
  • Ensure assigned direct care staff records are current
  • Ensure assigned service recipient clinical records are current
  • Conduct and document clinical and employee supervisory visits to ensure Medicaid and Abounds Policies/Procedures are followed
  • Conduct annual performance evaluations, direct care staff development plans, disciplinary meetings/documentation with direct care staff
  • Participate in required staff meetings
  • Participate and advocate for the recipient in the recipient’s ISP meetings
  • Develop individualized short term goals, task analysis and interventions for assigned recipients
  • Complete recipient’s annual skills and preference assessments (as applicable)
  • Ensure all RDSPs are completed (as applicable)
  • Ensure client specific training's are completed for every recipient/staff relationship
  • Enter goals into OnTarget
  • Complete TARs (as applicable) in Provider Direct before annual ISP
  • Review weekly billing to ensure hours are used or complete deviation of service forms
  • Complete quarterly progress reports
  • Review and update task analysis per recipient’s goal frequency
  • Ensure staffing patterns are covered and back up staff is available
  • Resolve and document recipient/guardian grievances and report to Supervisor
  • Complete significant event and incident reports within 24 hours (recipient and employee/contractor)
  • Schedule direct care staff/contractors to provide direct care. Hours must comply with service authorizations and waiver limitations
  • Assist the recipient and families with finding and procuring appropriate resources and services in the community
  • Represent Abound Health at Community Events

General Responsibilities:

  • Provide privacy and confidentiality of all participants’ records.
  • Assure participants are free from abuse, mistreatment, and/or neglect and report any incidents of such to supervisor.
  • Treat each participant with dignity and respect.
  • Develop and demonstrate a thorough understanding of Abounds philosophy and purpose.
  • Provide a positive atmosphere which facilitates growth and learning of coping skills.

  • Maintain Therapeutic working relationship with Service Recipients, Family Members, Team

Credentialing:

Must be Credentialed as a Qualified Professional with either:

  1. Graduate of a college/university with a Master’s degree in a related human service field and has 1 year of full-time IDD experience accumulated.
  2. Graduate of a college/university with a baccalaureate degree in a related human service field and has 2 years of full-time IDD experience accumulated.
  3. Graduate of a college/university with a baccalaureate degree in an unrelated human service field and has 4 years of full-time IDD experience accumulated.

Requirements:

  • At least 18 years of age
  • Valid Driver License
  • Safe Driving Record
  • Criminal, LEIE, NC Health Care Registry Check and Sexual Offender Background checks clear of health and safety risks
  • Reliable and safe transportation
  • Current Auto Insurance and Car Registration
  • Can speak Spanish preferred but not required
