Abound Health is seeking a Licensed Nutritionist or Dietitian (LDN, LN, RDN) to join our Special Consultation Services team in Charlotte, North Carolina (Hybrid). In this role, you will report to the Program Director. As a licensed Nutritionist or Dietitian, you will develop personalized meal plans and nutritional strategies for individuals with disabilities, promoting overall health and well-being. In this role, you will assess dietary needs, create treatment plans, and collaborate with caregivers and healthcare professionals to support individuals in achieving their nutritional goals. Key Responsibilities:

• Provide personalized nutrition assessments and intervention plans for clients in-home or via telehealth, ensuring alignment with individual health needs.
• Train and support family members, caregivers, and support staff on implementing nutrition-based interventions and meal planning.
• Monitor and evaluate client progress, adjusting nutrition plans as needed to ensure effectiveness and optimal health outcomes.
• Collaborate with interdisciplinary care teams to integrate nutrition into clients' overall care plans for holistic support.
• Maintain accurate documentation of nutrition services, adhering to relevant regulations, client rights, and confidentiality standards.

Qualifications:

• Must have experience in developing and implementing personalized nutrition intervention plans for individuals with intellectual and developmental disabilities.
• Must possess strong analytical, decision-making, interpersonal, and communication skills to effectively support clients, caregivers, and multidisciplinary teams.

Education:

• Valid licensure, registration, or certification specific to discipline (LDN, LN, RDN). Contractors will take action as necessary to maintain active licensure, registration or certification status, such as any continuing education requirements, mandatory training, and clinical consultation.

Compensation & Benefits:

The target base hourly range for is between $91 - $111/hr.
